Training Site Coordinator

An education professional designated or recognized by the Training Center that serves as the contact and person responsible for all aspects of an authorized Training Site, including operations, quality assurance and educational process. The Training Site Coordinator must have a valid provider and instructor card in at least one of the disciplines the Training Site is approved to teach.

Please note* All requests for instructor cards must meet the following requirements:

· Initial card requests: must be accompanied by a letter of course completion, core certificate and a completed monitoring form.

· Renewal card requests: must include copy of Instructor card, front and back. Plus, copies of 4-rosters from classes taught in the past 2-years.
