Non- Physician Registration Information

Pre-Registration: Pay fee listed (postmarked 14 days or more before start date)   
Late registration: add an additional $15.00 ( 13 days or less before class start date) MUST  SEND MONEY ORDER.
Registration @ the door: MUST PAY CASH  OR  MONEY ORDER (+ $15.00 late fee)

Additional Information:

Registration confirmation letter  will be mailed at least three weeks before the start of class and after completion of registration form and accompanying course fee.

Refund Policy: Written cancellations postmarked  up to 7 days prior to start of course will result in an administrative fee of  $25.00.  No refunds after 7 days prior to class.

No Shows: There is no refund for no-shows.
